REGISTRATION INFORMATION FOR PRESCHOOL THROUGH GRADE 6

TO REGISTER YOUR CHILD

According to state law, Hawaii Revised Statutes (HRS) §302A-1143 and Hawaii Administrative Rule (HAR) §8-13, school-aged children are required to attend the school in the area in which they (legal parent/guardian) reside.

  1. Fill out STUDENT ENROLLMENT FORMS (see REGISTRATION FORMS Section)

  2. TB CLEARANCE (PPD or MANTOUX) Must be done within 1 year of child starting school in Hawaii. TB must show Date Given, Date Read, Results in MM, & a dr's/nurse's signature/stamp.

  3. PHYSICAL EXAMINATION (Form 14) and IMMUNIZATION RECORDS. Must be done within 1 year of child starting school in Hawaii & must have a Dr's signature.

  4. Original BIRTH CERTIFICATE (we will make a copy and return your original back to you).

  5. PROOF OF RESIDENCY. Must be Utility Bill (ex. Telephone (not cellular), Cable, HECO, Water, etc.) or Lease or Rental Agreement. Residency should show LEGAL PARENT/GUARDIAN NAME. Bill must be current.

    • Parent/guardians need to be informed that, according to HRS§710-1063, falsification on a government application is a misdemeanor and that when such a violation is found, the child will return to the school where he/she should properly be attending.

  6. If student is transferring from another public school, parent must bring Student's Release from previous public school, a proof of residency, and fill out enrollment forms.
